[ Upstream commit d64bf89a75b65f83f06be9fb8f978e60d53752db ]

rds_ibdev:ipaddr_list and rds_ibdev:conn_list are initialized
after allocation some resources such as protection domain.
If allocation of such resources fail, then these uninitialized
variables are accessed in rds_ib_dev_free() in failure path. This
can potentially crash the system. The code has been updated to
initialize these variables very early in the function.

refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as
a reference counter. This allows to avoid accidental
refcounter overflows that might lead to use-after-free
situations.

The function rds_trans_register always returns 0. As such, it is not
necessary to check the returned value.

are used only in the ib.c file. As such, the static type is
added to limit them in this file.

Instead of exposing ib_get_dma_mr to ULPs and letting them use it more or
less unchecked, this moves the capability of creating a global rkey into
the RDMA core, where it can be easily audited. It also prints a warning
everytime this feature is used as well.

The ->sk_user_data contains a pointer to the rds_conn_path
for the socket. Use this consistently in the rds_tcp_data_ready
callbacks to get the rds_conn_path for rds_recv_incoming.
