'0' is now used as the default cmd_per_lun value,
so there's no need to explicitly set it to '1' in the
host template.

Using seq_printf to print a simple string is a lot more expensive than
it needs to be, since seq_puts exists. Replace seq_printf with
seq_puts when possible.

The macro SPRINTF doesn't save a lot of typing or make the code more
readable, and depending on a specific identifier (m) in the
surrounding scope is generally frowned upon. Nuke it.

Remove all uncommented debugging code and move all
printk() statements over to dev_printk().
And while we're at it we should be doing a whitespace
cleanup, too.

The SCSI standard defines 64-bit values for LUNs, and large arrays
employing large or hierarchical LUN numbers become more and more
common.

So update the linux SCSI stack to use 64-bit LUN numbers.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

which also takes care about the following warning during compilation time:
