This driver has been mostly rewritten since Michael Brown's initial
work, so swap the order of the authors.

This integrates support for the SFC9000 family of 10G Ethernet
controllers and LAN-on-motherboard chips, starting with the SFL9021
'Siena' and SFC9020 'Bethpage'.

Only some PHYs have firmware support for a LED blink mode, so we
currently blink the others in a timer function. Since all PHYs have
simple on and off modes, we don't gain anything by using multiple
blink implementations. Also, since we have a process context there
is no need to use a timer.

The 'XFP' driver is really a driver for the QT2022C2 and QT2025C PHYs,
covering both more and less than XFP. Rename its functions and
constants to reflect reality and to reduce namespace pollution when
sfc is a built-in driver.

This is a new PHY supporting SFP+ modules, used in the SFN4112F
reference design. It is similar to the QT2022C2 and shares much of
its support code.

During SFN4111T initialisation, check whether the PHY boot status
indicates a bad firmware checksum. If so, prepare to reflash rather
than continuing with normal initialisation.

Remove redundant PHY boot status check from tenxpress_phy_init().

<title>sfc: SFX7101: Remove workaround for bad link training</title>

Early versions of the SFX7101 firmware could complete link training in
a state where it would not adequately cancel noise (Solarflare bug
10750). We previously worked around this by resetting the PHY after
seeing many Ethernet CRC errors. This workaround is unsafe since it
takes no account of the interval between errors; it also appears to
be unnecessary with production firmware. Therefore remove it.

Add type codes for the new PHY and rename the SFX7101 type code.

Add definition of clause 22 extension MMD.

Adapt the 10Xpress SFX7101 code to support the SFT9001 as well.
Clean up register definitions.

This allows updating PHY firmware for one interface without removing
all other interfaces handled by the driver.

Replace tx_disabled flags and 10Xpress status enumeration with flags in
enum efx_phy_mode.

Prevent an interface from being brought up while in PHY flash mode.
