The functions that add an async subdev to an async subdev notifier take
as an argument the size of the container structure they need to
allocate. This is error prone, as passing an invalid size will not be
caught by the compiler. Wrap those functions in macros that take a
container type instead of a size, and cast the returned pointer to the
desired type. The compiler will catch mistakes if the incorrect type is
passed to the macro, as the assignment types won't match.

Change v4l2_async_notifier_add_fwnode_remote_subdev semantics
so it allocates the struct v4l2_async_subdev pointer.

This makes the API consistent: the v4l2-async subdevice addition
functions have now a unified usage model. This model is simpler,
as it makes v4l2-async responsible for the allocation and release
of the subdevice descriptor, and no longer something the driver
has to worry about.

On the user side, the change makes the API simpler for the drivers
to use and less error-prone.

in_interrupt() covers hard and soft interrupt servicing and bottom half
disabled contexts, which is semantically undefined and useless.

The comment for __ccdc_lsc_configure() "Context: in_interrupt()" is
therefore as useful as "Context: unknown'. Remove it.

After mm.h was removed from the asm-generic version of cacheflush.h,
s390 allyesconfig shows several warnings of the following nature:

  In file included from arch/s390/include/generated/asm/cacheflush.h:1,
                   from drivers/media/platform/omap3isp/isp.c:42:
include/asm-generic/cacheflush.h:16:42: warning: 'struct mm_struct' declared inside parameter list will not be visible outside of this definition or declaration

As Geert and Laurent point out, this driver does not need this header in
the two files that include it. Remove it so there are no warnings.

This seems to lead to some crazy include loops when using
asm-generic/cacheflush.h on more architectures, so leave it to the arch
header for now.
