The kvmalloc function fails with a warning if the size is larger than
INT_MAX. Linus said that there should be limits that prevent this warning
from being hit. This commit adds the limits to the dm-stats subsystem
in DM core.

<title>dm: Annotate struct dm_stat with __counted_by</title>

Prepare for the coming implementation by GCC and Clang of the __counted_by
attribute. Flexible array members annotated with __counted_by can have
their accesses bounds-checked at run-time checking via CONFIG_UBSAN_BOUNDS
(for array indexing) and CONFIG_FORTIFY_SOURCE (for strcpy/memcpy-family
functions).

As found with Coccinelle[1], add __counted_by for struct dm_stat.

Check alloc_precpu()'s return value and return an error from
dm_stats_init() if it fails. Update alloc_dev() to fail if
dm_stats_init() does.

Otherwise, a NULL pointer dereference will occur in dm_stats_cleanup()
even if dm-stats isn't being actively used.
