[ Upstream commit 6b6e2e8fd0de3fa7c6f4f8fe6841b01770b2e7bc ]

The `pmbus_identify()` function fails to correctly determine the number
of supported pages on PMBus devices. This occurs because `info->pages`
is implicitly zero-initialised, and `pmbus_set_page()` does not perform
writes to the page register if `info->pages` is not yet initialised.
Without this patch, `info->pages` is always set to the maximum after
scanning.

This patch initialises `info->pages` to `PMBUS_PAGES` before the probing
loop, enabling `pmbus_set_page()` writes to make it out onto the bus
correctly identifying the number of pages. `PMBUS_PAGES` seemed like a
reasonable non-zero number because that's the current result of the
identification process.

Testing was done with a PMBus device in QEMU.

[ Upstream commit a5436af598779219b375c1977555c82def1c35d0 ]

If there is an input undervoltage fault, reported in STATUS_INPUT
command response, there is quite likely a "Unit Off For Insufficient
Input Voltage" condition as well.

Add a constant for bit 3 of STATUS_INPUT. Update the Vin limit
attributes to include both bits in the mask for clearing faults.

If an input undervoltage fault occurs, causing a unit off for
insufficient input voltage, but the unit is off bit is not cleared, the
STATUS_WORD will not be updated to clear the input fault condition.
Including the unit is off bit (bit 3) allows for the input fault
condition to completely clear.

On PMBUS devices with multiple pages, the regulator ops need to be
protected with the update mutex. This prevents accidentally changing
the page in a separate thread while operating on the PMBUS_OPERATION
register.

Tested on Infineon xdpe11280 while a separate thread polls for sensor
data.

[ Upstream commit 6d1d41c075a1a54ba03370e268171fec20e06563 ]

Issue:
When PEC is enabled, binding adm1272 to the adm1275 would
fail due to PEC error. See below:
adm1275: probe of xxxx failed with error -74

Diagnosis:
Per the datasheet of adm1272, adm1278, adm1293 and amd1294,
PMON_CONFIG (0xd4) is 16bits wide. On the other hand,
PMON_CONFIG (0xd4) for adm1275 is 8bits wide. The driver should not
assume everything is 8bits wide and read only 8bits from it.

Solution:
If it is adm1272, adm1278, adm1293 and adm1294, use i2c_read_word.
Else, use i2c_read_byte

Testing:
Binding adm1272 to the driver.
The change is only tested on adm1272.

commit cf2b012c90e74e85d8aea7d67e48868069cfee0c upstream.

Change 21537dc driver PMBus polling of MFR_COMMON from bits 5/4 to
bits 6/5. This fixs a LTC297X family bug where polling always returns
not busy even when the part is busy. This fixes a LTC388X and
LTM467X bug where polling used PEND and NOT_IN_TRANS, and BUSY was
not polled, which can lead to NACKing of commands. LTC388X and
LTM467X modules now poll BUSY and PEND, increasing reliability by
eliminating NACKing of commands.

commit 92b39ad440968bab38eb6577d63c12994601ed94 upstream.

The LED blink_set function incorrectly did not tell the PSU LED to blink
if brightness was LED_OFF. Fix this, and also correct the LED_OFF
command data, which should give control of the LED back to the PSU
firmware. Also prevent I2C failures from getting the driver LED state
out of sync and add some dev_dbg statements.
