[ Upstream commit 00312419f0863964625d6dcda8183f96849412c6 ]

On systems using the hash MMU, there is a software SLB preload cache that
mirrors the entries loaded into the hardware SLB buffer. This preload
cache is subject to periodic eviction — typically after every 256 context
switches — to remove old entry.

To optimize performance, the kernel skips switch_mmu_context() in
switch_mm_irqs_off() when the prev and next mm_struct are the same.
However, on hash MMU systems, this can lead to inconsistencies between
the hardware SLB and the software preload cache.

If an SLB entry for a process is evicted from the software cache on one
CPU, and the same process later runs on another CPU without executing
switch_mmu_context(), the hardware SLB may retain stale entries. If the
kernel then attempts to reload that entry, it can trigger an SLB
multi-hit error.

The following timeline shows how stale SLB entries are created and can
cause a multi-hit error when a process moves between CPUs without a
MMU context switch.

CPU 0                                   CPU 1

&gt;&gt;From the above analysis, during early exec the hardware SLB is cleared,
and entries from the software preload cache are reloaded into hardware
by switch_slb. However, preload_new_slb_context and slb_setup_new_exec
also attempt to load some of the same entries, which can trigger a
multi-hit. In most cases, these additional preloads simply hit existing
entries and add nothing new. Removing these functions avoids redundant
preloads and eliminates the multi-hit issue. This patch removes these
two functions.

We tested process switching performance using the context_switch
benchmark on POWER9/hash, and observed no regression.

Without this patch: 129041 ops/sec
With this patch:    129341 ops/sec

We also measured SLB faults during boot, and the counts are essentially
the same with and without this patch.

SLB faults without this patch: 19727
SLB faults with this patch: 19786

[ Upstream commit eae40a6da63faa9fb63ff61f8fa2b3b57da78a84 ]

HPTE format was changed since Power9 (ISA 3.0) onwards. While dumping
kernel hash page tables, nothing gets printed on powernv P9+. This patch
utilizes the helpers added in the patch tagged as fixes, to convert new
format to old format and dump the hptes. This fix is only needed for
native_find() (powernv), since pseries continues to work fine with the
old format.

commit 7b31f7dadd7074fa70bb14a53bd286ffdfc98b04 upstream.

For both CONFIG_NUMA enabled/disabled use mem_topology_setup() to
update max/min_low_pfn.

This also adds min_low_pfn update to CONFIG_NUMA which was initialized
to zero before. (mpe: Though MEMORY_START is == 0 for PPC64=y which is
all possible NUMA=y systems)

This is due to virt_addr_valid() using high_memory before it is set.

high_memory is set in mem_init() using max_low_pfn, but max_low_pfn
is available long before, it is set in mem_topology_setup(). So just
like commit daa9ada2093e ("powerpc/mm: Fix boot crash with FLATMEM")
moved the setting of max_mapnr immediately after the call to
mem_topology_setup(), the same can be done for high_memory.

[ Upstream commit 4a7aee96200ad281a5cc4cf5c7a2e2a49d2b97b0 ]

In kasan_init_region, when k_start is not page aligned, at the begin of
for loop, k_cur = k_start &amp; PAGE_MASK is less than k_start, and then
`va = block + k_cur - k_start` is less than block, the addr va is invalid,
because the memory address space from va to block is not alloced by
memblock_alloc, which will not be reserved by memblock_reserve later, it
will be used by other places.

As a result, memory overwriting occurs.

for example:
int __init __weak kasan_init_region(void *start, size_t size)
{
[...]
	/* if say block(dcd97000) k_start(feef7400) k_end(feeff3fe) */
	block = memblock_alloc(k_end - k_start, PAGE_SIZE);
	[...]
	for (k_cur = k_start &amp; PAGE_MASK; k_cur &lt; k_end; k_cur += PAGE_SIZE) {
		/* at the begin of for loop
		 * block(dcd97000) va(dcd96c00) k_cur(feef7000) k_start(feef7400)
		 * va(dcd96c00) is less than block(dcd97000), va is invalid
		 */
		void *va = block + k_cur - k_start;
		[...]
	}
[...]
}

Therefore, page alignment is performed on k_start before
memblock_alloc() to ensure the validity of the VA address.

[ Upstream commit f46c8a75263f97bda13c739ba1c90aced0d3b071 ]

kasprintf() returns a pointer to dynamically allocated memory
which can be NULL upon failure. Ensure the allocation was successful
by checking the pointer validity.

A bisect pointed to the breakage beginning with commit 9fee28baa601 ("powerpc:
implement the new page table range API").

Analysis of the oops pointed to a struct page with a corrupted
compound_head being loaded via page_folio() -&gt; _compound_head() in
hash_page_do_lazy_icache().

The access by the mpic code is to an MMIO address, so the expectation
is that the struct page for that address would be initialised by
init_unavailable_range(), as pointed out by Aneesh.

Instrumentation showed that was not the case, which eventually lead to
the realisation that pfn_valid() was returning false for that address,
causing the struct page to not be initialised.

Because the system is using FLATMEM, the version of pfn_valid() in
memory_model.h is used:

static inline int pfn_valid(unsigned long pfn)
{
	...
	return pfn &gt;= pfn_offset &amp;&amp; (pfn - pfn_offset) &lt; max_mapnr;
}

Which relies on max_mapnr being initialised. Early in boot max_mapnr is
zero meaning no PFNs are valid.

max_mapnr is initialised in mem_init() called via:

  start_kernel()
    mm_core_init()  # init/main.c:928
      mem_init()

But that is too late for the usage in init_unavailable_range() called via:

  start_kernel()
    setup_arch()    # init/main.c:893
      paging_init()
        free_area_init()
          init_unavailable_range()

Although max_mapnr is currently set in mem_init(), the value is actually
already available much earlier, as soon as mem_topology_setup() has
completed, which is also before paging_init() is called. So move the
initialisation there, which causes paging_init() to correctly initialise
the struct page and fixes the bug.

This bug seems to have been lurking for years, but went unnoticed
because the pre-folio code was inspecting the uninitialised page-&gt;flags
but not dereferencing it.

Thanks to Erhard and Aneesh for help debugging.

[ Upstream commit 1aa000667669fa855853decbb1c69e974d8ff716 ]

By adding a forward declaration for struct lppaca we can untangle paca.h
and lppaca.h. Also move get_lppaca() into lppaca.h for consistency.

Add includes of lppaca.h to some files that need it.

<title>powerpc/kasan: Disable KCOV in KASAN code</title>

[ Upstream commit ccb381e1af1ace292153c88eb1fffa5683d16a20 ]

As per the generic KASAN code in mm/kasan, disable KCOV with
KCOV_INSTRUMENT := n in the makefile.

This fixes a ppc64 boot hang when KCOV and KASAN are enabled.
kasan_early_init() gets called before a PACA is initialised, but the
KCOV hook expects a valid PACA.
