<title>maccess: make get_kernel_nofault() check for minimal type compatibility</title>

Now that we've renamed probe_kernel_address() to get_kernel_nofault()
and made it look and behave more in line with get_user(), some of the
subtle type behavior differences end up being more obvious and possibly
dangerous.

When you do

        get_user(val, user_ptr);

the type of the access comes from the "user_ptr" part, and the above
basically acts as

        val = *user_ptr;

by design (except, of course, for the fact that the actual dereference
is done with a user access).

Note how in the above case, the type of the end result comes from the
pointer argument, and then the value is cast to the type of 'val' as
part of the assignment.

So the type of the pointer is ultimately the more important type both
for the access itself.

But 'get_kernel_nofault()' may now _look_ similar, but it behaves very
differently.  When you do

        get_kernel_nofault(val, kernel_ptr);

it behaves like

        val = *(typeof(val) *)kernel_ptr;

except, of course, for the fact that the actual dereference is done with
exception handling so that a faulting access is suppressed and returned
as the error code.

But note how different the casting behavior of the two superficially
similar accesses are: one does the actual access in the size of the type
the pointer points to, while the other does the access in the size of
the target, and ignores the pointer type entirely.

Actually changing get_kernel_nofault() to act like get_user() is almost
certainly the right thing to do eventually, but in the meantime this
patch adds logit to at least verify that the pointer type is compatible
with the type of the result.

In many cases, this involves just casting the pointer to 'void *' to
make it obvious that the type of the pointer is not the important part.
It's not how 'get_user()' acts, but at least the behavioral difference
is now obvious and explicit.

Better describe what this helper does, and match the naming of
copy_from_kernel_nofault.

Also switch the argument order around, so that it acts and looks
like get_user().

<title>powerpc: Use a datatype for instructions</title>

Currently unsigned ints are used to represent instructions on powerpc.
This has worked well as instructions have always been 4 byte words.

However, ISA v3.1 introduces some changes to instructions that mean
this scheme will no longer work as well. This change is Prefixed
Instructions. A prefixed instruction is made up of a word prefix
followed by a word suffix to make an 8 byte double word instruction.
No matter the endianness of the system the prefix always comes first.
Prefixed instructions are only planned for powerpc64.

Introduce a ppc_inst type to represent both prefixed and word
instructions on powerpc64 while keeping it possible to exclusively
have word instructions on powerpc32.

<title>powerpc: Use a macro for creating instructions from u32s</title>

In preparation for instructions having a more complex data type start
using a macro, ppc_inst(), for making an instruction out of a u32. A
macro is used so that instructions can be used as initializer elements.
Currently this does nothing, but it will allow for creating a data type
that can represent prefixed instructions.

<title>powerpc: Activate CONFIG_THREAD_INFO_IN_TASK</title>

This patch activates CONFIG_THREAD_INFO_IN_TASK which
moves the thread_info into task_struct.

Moving thread_info into task_struct has the following advantages:
  - It protects thread_info from corruption in the case of stack
    overflows.
  - Its address is harder to determine if stack addresses are leaked,
    making a number of attacks more difficult.

This has the following consequences:
  - thread_info is now located at the beginning of task_struct.
  - The 'cpu' field is now in task_struct, and only exists when
    CONFIG_SMP is active.
  - thread_info doesn't have anymore the 'task' field.

This patch:
  - Removes all recopy of thread_info struct when the stack changes.
  - Changes the CURRENT_THREAD_INFO() macro to point to current.
  - Selects CONFIG_THREAD_INFO_IN_TASK.
  - Modifies raw_smp_processor_id() to get -&gt;cpu from current without
    including linux/sched.h to avoid circular inclusion and without
    including asm/asm-offsets.h to avoid symbol names duplication
    between ASM constants and C constants.
  - Modifies klp_init_thread_info() to take a task_struct pointer
    argument.

When I had lockdep turned on and dropped into kgdb I got a nice splat
on my system. Specifically it hit:

Looking closely at it, it seems like a really bad idea to be calling
local_irq_enable() in kgdb_roundup_cpus(). If nothing else that seems
like it could violate spinlock semantics and cause a deadlock.

Instead, let's use a private csd alongside
smp_call_function_single_async() to round up the other CPUs. Using
smp_call_function_single_async() doesn't require interrupts to be
enabled so we can remove the offending bit of code.

In order to avoid duplicating this across all the architectures that
use the default kgdb_roundup_cpus(), we'll add a "weak" implementation
to debug_core.c.

Looking at all the people who previously had copies of this code,
there were a few variants. I've attempted to keep the variants
working like they used to. Specifically:
* For arch/arc we passed NULL to kgdb_nmicallback() instead of
  get_irq_regs().
* For arch/mips there was a bit of extra code around
  kgdb_nmicallback()

NOTE: In this patch we will still get into trouble if we try to round
up a CPU that failed to round up before. We'll try to round it up
again and potentially hang when we try to grab the csd lock. That's
not new behavior but we'll still try to do better in a future patch.

The function kgdb_roundup_cpus() was passed a parameter that was
documented as:

&gt; the flags that will be used when restoring the interrupts. There is
&gt; local_irq_save() call before kgdb_roundup_cpus().

Nobody used those flags. Anyone who wanted to temporarily turn on
interrupts just did local_irq_enable() and local_irq_disable() without
looking at them. So we can definitely remove the flags.
