[ Upstream commit 66e29fcda1824f0427966fbee2bd2c85bf362c82 ]

With idle polling, IPIs are not sent when a CPU idle, but queued
and run later from do_idle(). The default kgdb_call_nmi_hook()
implementation gets the pointer to struct pt_regs from get_irq_reqs(),
which doesn't work in that case because it was not called from the
IPI interrupt handler. Fix it by defining our own kgdb_roundup()
function which sents an IPI_ENTER_KGDB. When that IPI is received
on the target CPU kgdb_nmicallback() is called.

No other architecture has setup_profiling_timer() in the init section,
thus on parisc we face this section mismatch warning:

Apart from adding the helper function itself, the rest of the kernel is
converted mechanically using:

  git grep -l 'atomic_inc.*mm_count' | xargs sed -i 's/atomic_inc(&amp;\(.*\)-&gt;mm_count);/mmgrab\(\1\);/'
  git grep -l 'atomic_inc.*mm_count' | xargs sed -i 's/atomic_inc(&amp;\(.*\)\.mm_count);/mmgrab\(\&amp;\1\);/'

This is needed for a later patch that hooks into the helper, but might
be a worthwhile cleanup on its own.

(Michal Hocko provided most of the kerneldoc comment.)

<title>arch/hotplug: Call into idle with a proper state</title>

Let the non boot cpus call into idle with the corresponding hotplug state, so
the hotplug core can handle the further bringup. That's a first step to
convert the boot side of the hotplugged cpus to do all the synchronization
with the other side through the state machine. For now it'll only start the
hotplug thread and kick the full bringup of the cpu.

Remove the function smp_send_start() that is not used anywhere.

This was partially found by using a static code analysis program called cppcheck.

The number of IPI calls is already visible as per-cpu IPI irq counters
in/proc/cpuinfo, so let's drop this additional counting.

Commit 9a46ad6d6df3b54 "smp: make smp_call_function_many() use logic
similar to smp_call_function_single()" has unified the way to handle
single and multiple cross-CPU function calls. Now only one interrupt
is needed for architecture specific code to support generic SMP function
call interfaces, so kill the redundant single function call interrupt.
