The ia64 build generates many warnings like this:

WARNING: EXPORT symbol "empty_zero_page" [vmlinux] version generation failed, symbol will not be versioned.

Besides adding the necessary header this also requires fiddling with
some explicit .S -> .o rules.

arch, mm: convert all architectures to use 5level-fixup.h

If an architecture uses 4level-fixup.h we don't need to do anything as
it includes 5level-fixup.h.

If an architecture uses pgtable-nop*d.h, define __ARCH_USE_5LEVEL_HACK
before inclusion of the header. It makes asm-generic code to use
5level-fixup.h.

If an architecture has 4-level paging or folds levels on its own,
include 5level-fixup.h directly.

prefetch_stack() is defined by IA64, but not actually used anywhere anymore.

Remove it.

Introduce a trivial, mostly empty <linux/sched/cputime.h> header
to prepare for the moving of cputime functionality out of sched.h.

Update all code that relies on these facilities.

Update code that relied on sched.h including various MM types for them.

This will allow us to remove the <linux/mm_types.h> include from <linux/sched.h>.

We are going to split <linux/sched/task_stack.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.

Create a trivial placeholder <linux/sched/task_stack.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

Include the new header in the files that are going to need it.
